Once you have made your initial appointment, you can fill out the new patient packet ahead of time!
- Download, print and complete the form below. Doing so prior to your visit will help speed up the process when you come to the office.
Established patients can update forms online through our patient portal. If you need help accessing the portal, please call the office and we’ll be happy to help.
What to Expect
During your initial visit, you and the doctor will discuss your personal and family medical history and risk factors for disease. The doctor will listen to your symptoms, perform a comprehensive physical examination, review any diagnostic tests previously performed (if applicable), and answer any questions you might have. If necessary, the doctor will refer you for initial or additional testing, including blood work or diagnostic scans, or perform it in-house if available.
Once your podiatrist makes a diagnosis, he or she will explain your treatment options and any prescription medications to be ordered. If the doctor recommends surgery, he or she will discuss this with you and refer you to our surgery scheduler.

How to Prepare
To prepare for your consultation with your foot and ankle doctor, please be sure to bring the following with you to your appointment:
- List of your symptoms and any questions you have for the doctor
- Insurance card
- Identification
- List of medications you take regularly and any previous surgeries you’ve had
- Registration forms (complete our online New Patient Packet)
- Referral from your primary care physician (if insurance requires one)
- Diagnostic films or other test results that may have been ordered or performed by another physician
